Adding fields to contact form is really hard job, and even if I did it on your installation it will took lot of time and checking :-)
I didn't test any contact form plugin, so you are free to do any tests you like and report back :-) We have default styles for form inputs, so if you disable plugin styles it should look nice in most cases.
