contact form widget error

it's me again
I have another question.
The contact form widget looks strange on my site with chrome, IE and Safari.
there is a big blank space between the title and the first input.
the dots are not in front of the input field.

As mentioned in documentation not all widgets may look good in footer sidebar, and contact is one of those :-)
