Add phone number and more options in Contact Form

Can you please tell me how can I add these fields in contact form? "Telephone Number", "Address" and "City"

Sorry it is too much to edit to make it good. If you like you can mess in airlock/advance/apollo13.php line ~1395 and edit function

function contact_form

Everything from contact form is there, and all you need is to copy and rename similiar fields in form and in part in sending mail.

Can you please add for me the phone number option? I need it for a client, I tried to mess around but I am not advanced enough to make it happen.
